RESUMO

INTRODUCTION: Technologies such as health and fitness applications (apps) and wearable activity trackers have recently gained popularity and may play a key role in promoting physical activity and reducing sedentary behaviours. Although several systematic reviews have investigated their efficacy in physical activity and sedentary behaviours, few studies have focused on their impact on work-related outcomes among workers. Here, to explore the effects of mHealth interventions designed to encourage physical activity and decrease sedentary behaviours on work-related outcomes, including absenteeism, presenteeism, productivity, work performance and workability among workers, we will conduct a systematic review based on recent articles and an extensive literature search. METHODS AND ANALYSIS: The literature search will be performed using PubMed, Web of Science, the Cochrane Library and the Japan Medical Abstracts Society from inception to 23 September 2023. We will select studies that (1) investigated the impact of mHealth interventions to promote physical activity and reduce sedentary behaviours on work-related outcomes such as absenteeism, presenteeism, productivity, work performance and workability; (2) were designed as a randomised controlled trial (RCT) or non-randomised study of interventions (NRSI); (3) were conducted among workers and (4) were published as full-text original articles in Japanese or English. We will assess the review quality with the AMSTAR 2 tool. The risk of bias will be assessed with the RoB tool 2.0 and ROBINS-I. ETHICS AND DISSEMINATION: Ethical approval is unnecessary as the study will rely solely on previously published articles. The research results will be submitted for publication in a peer-reviewed scientific journal. TRIAL REGISTRATION NUMBER: The study protocol has been registered with the UMIN Clinical Trials Registry (ID=UMIN000052290).

BACKGROUND: In Japan, routine administration to one-year-old children of two-dose immunization for varicella was introduced in October 2014. Object The object of this study was to report outbreaks of varicella under routine immunization at a nursery school and in its surrounding area using data of surrounding areas from the (Nursery) School Absenteeism Surveillance System. Then, we measured the effectiveness of routine two-dose immunization for varicella to onset. We tentatively assessed its severity in a nursery school. METHOD: The study period extended from April 2017 through March 2018. The study area comprised Nursery school B and other nursery schools, and elementary and junior high schools in City A. Subjects in Nursery school B were 120 children. We analyzed vaccine effectiveness (VE) as an observational study and assessed severity using Fisher's exact test. We also assessed VE for severity using linear regression. Severity was defined as the length of nursery school absence attributable to varicella infection. RESULTS: During the one month preceding a period of two weeks before the initial case at Nursery school B, there were 16 cases of varicella infection in nursery schools, 45 cases in elementary schools, and one case in junior high schools in City A. For children who had received one vaccine dose or more, VE was 48.1% for all ages and 49.2% among children three years old and older. No significant VE against infection was found. Vaccination using one dose or more can reduce severity significantly. DISCUSSION AND CONCLUSION: Because many nursery school children who had received two doses of vaccine were infected, VE was estimated as low in the nursery school and not significant. Although VE for severity with more than one dose was confirmed, a second dose might not reduce severity compared to one dose.

Clinical practice guidelines (CPGs) are used by both healthcare users and providers, so their recognition is important. The present study's purpose was to clarify the features of healthcare users who are aware of the CPGs. A cross-sectional survey was conducted targeting Internet survey panels (n = 6000). The study participants (age range, 20s to 60s) had no medical qualifications and had received medical care in the last 3 months. Multivariate logistic regression analysis was performed to clarify the factors related to the awareness of CPGs. When "I have seen the CPGs" was used as the objective variable, the odds ratios (ORs) were high for "e-health literacy/score 31-40" (OR = 8.72, 95% confidence interval [CI]: 6.51-11.68), "Sources of health information/healthcare workers and professionals" (OR = 2.61, 95% CI: 2.17-3.14), "Age/20s" (OR = 2.38, 95% CI: 1.74-3.23), and "I have been diagnosed and treated for a major illness" (OR = 2.01, 95% CI: 1.52-2.65). These results could be applied to aid the dissemination and utilization of CPGs among healthcare users.

Control of infectious diseases requires switching from usual hygiene such as water wiping and cleaning, to control measures including appropriate and aggressive disinfection using 70% alcohol and/or hypochlorous acid depending on the pathogen. Nevertheless, some nursery schools might not understand or select proper disinfectant methods. A survey was administered by the local government of Ibaraki prefecture, Japan in January 2018 to all 456 nursery schools in Ibaraki prefecture. The surveyed items were the number of nursery teachers, usual manual hygiene among children, usual disinfection for lavatories, diaper changing spaces, tables used by children for meals, and classroom floors when there was no vomiting in the facilities and no outbreak in surrounding area. Moreover, it asked about disinfection procedures if children vomited during a community outbreak of gastroenteritis infection. We defined proper use for usual disinfection of a lavatory or diaper changing space as chlorine-based disinfectant including hypochlorous acid or 70% alcohol according to guidelines. Overall, 403 nursery schools responded to the survey. All nursery schools implemented usual hand hygiene. Association between proper disinfection and the size of nursery schools was not significant. Moreover, association between proper disinfection and nursery schools with nurse presence was not found to be significant.

OBJECTIVES: This study aims to develop a scale of "women-doctor-friendly working conditions in a hospital setting". METHODS: A task team consisting of relevant people including a medical doctor and a hospital personnel identified 36 items related to women-doctor-friendly working conditions. From December in 2012 to January in 2013, we sent a self-administered questionnaire to 807 full-time employees including faculty members and medical doctors who worked for a university-affiliated hospital. We asked them to score the extent to which they think it is necessary for women doctors to balance between work and gender role responsibilities on the basis of the Likert scale. We carried out a factor analysis and computed Cronbach's alpha to develop a scale and investigated its construct validity and reliability. RESULTS: Of the 807 employees, 291 returned the questionnaires (response rate, 36.1%). The item-total correlation (between an individual item score and the total score) coefficient was in the range from 0.44 to 0.68. In factor analysis, we deleted six items, and five factors were extracted on the basis of the least likelihood method with the oblique Promax rotation. The factors were termed "gender equality action in an organization", "the compliance of care leave in both sexes and parental leave in men", "balance between life events and work", "childcare support at the workplace", and "flexible employment status". The Cronbach's alpha values of all the factors and the total items were 0.82-0.89 and 0.93, respectively, suggesting that the scale we developed has high reliability. CONCLUSIONS: The result indicated that the scale of women-doctor-friendly working conditions consisting of five factors with 30 items is highly validated and reliable.

BACKGROUND: Physicians are expected to perform three unique roles as a clinician, educator, and researcher in university hospitals. However, the actual practices of physicians performing different duties are relatively unknown. Therefore, the authors conducted an observational study at a university hospital to examine physicians' work activities. METHODS: Between 2011 and 2013, ten observers shadowed 20 physicians from different specialties for a day at the Tokyo Women's Medical University Hospital. Observers recorded physicians' activities every 30 seconds that were subsequently categorized into work types. The number of work types and activity changes performed by a physician in one observational period were counted. RESULTS: Authors categorized physicians' work activities into five groups: patient care (direct and indirect), education, research, professional development, and administration. All physicians performed at least one type of activity in addition to patient care. Activity change occurred 1.86 times per hour, on average. The median time-distribution of 20 physicians was 173.8 minutes, 213.8 minutes, 3.3 minutes, 5.0 minutes, 0 minutes, and 0.8 minutes for direct patient care, indirect patient care, education, research, professional development, and administration, respectively. CONCLUSION: Japanese hospital physicians performed multiple work duties including professional development and administrative activities in addition to triple duties.

Women's employment in Japan has increased substantially in recent decades, however little large scale research has been done on the impact of various types of working conditions on women's health. The aim of this study was to assess the menstrual cycle and menstrual pain problems of female workers and to investigate the factors that relate to them. The questionnaire was distributed to 8,150 women and 2,166 responded (26.6%). An anonymous self-administered questionnaire was used to get information about demographics, menstrual cycle status, the degree of menstrual pain, and employment and environmental factors. Irregular cycle menstruation was experienced in 17.1% of responded workers. We discovered the relationship between irregular menstrual cycles and stress, smell of cigarettes, age and smoking habits. Some degree of menstrual pain was experienced in 77.6% of responded workers. This study showed the relationship between menstrual pain and stress, high temperature and humidity, age, BMI, and number of births. In conclusion, we found that stress is thought to be an important factor related with menstrual cycle irregularities and menstrual pain among Japanese female workers.

Cutaneous body image, defined as the individual's mental perception of the appearance of their skin, hair and nails, is an important psychodermatological element in skin diseases. To measure individuals' cutaneous body image, a practical and accurate instrument is necessary. In this study, we translated the Cutaneous Body Image Scale (CBIS), a 7-item instrument originally created by Gupta et al. in 2004, into Japanese using a forward- and back-translation method and evaluated the reliability and validity of the instrument by psychometric tests. A total of 298 healthy adults (64 men and 234 women, aged 28.9 +/- 9.9 years) and 165 dermatology patients (56.7% eczema/dermatitis, 9.8% acne, 7.5% alopecia, 6.9% psoriasis, 19.1% skin tumor/fleck/other) (30 men and 135 women, aged 37.9 +/- 15.2 years) responded to the Japanese version of the CBIS. The internal-consistency reliability of the instrument was high (Cronbach's alpha, healthy adults 0.88, patients 0.84). The CBIS measure demonstrates good test-retest reliability (healthy adults gamma = 0.92, P < 0.0001; patients gamma = 0.79, P < 0.001). Compared to the healthy adults (4.11 +/- 1.80), the CBIS scores among dermatology patients (3.18 +/- 1.69, P = 0.000) were significantly low. The CBIS scores showed moderate correlation with the "emotions" and "global" scores of Skindex-16 in healthy adults (gamma = -0.397 and -0.373, respectively) and in patients (gamma = -0.431 and -0.38, respectively). A stepwise multiple regression analysis revealed that an emotional aspect of skin-condition related quality of life was the best predictor of cutaneous body image in both healthy adults and patients (beta = -0.31 and -0.41, respectively) followed by "body dissatisfaction" (beta = -0.17, and -0.23, respectively). Adjusted R(2) was 0.246 in healthy adults and 0.264 in patients. These were consistent with the results from the original the CBIS. These results suggest that the Japanese version of the CBIS is a reliable and valid instrument to measure the cutaneous body image of Japanese adults and also dermatology patients.

OBJECTIVE: Airway cyclooxygenase-2 (COX-2) is induced by cytokine-mediated inflammation such as occurs in asthma. However, the role of COX-2 in the pathophysiology of asthma is not fully understood. METHODS: Allergic inflammation, airway responsiveness to methacholine and mucous cell metaplasia after ovalbumin sensitization in the airways of COX-2 deficient (-/-) mice, COX-2 (+/+) mice and C57BL/6J mice treated with a selective COX-2 inhibitor, nimesulide were assessed. Histology, cell analysis, measurements of arachidonic acid metabolites and Th2 cytokine levels in bronchoalveolar lavage fluid (BALF), and measurement of serum IgE were performed. RESULTS: Eosinophil infiltration into the airway wall, and the number of eosinophils in BALF were greater in sensitized COX-2 (-/-) mice than in sensitized COX-2 (+/+) mice. The levels of cysteinyl leukotrienes (LTC4/D4/E4), prostaglandin E2 (PGE2) and interleukin (IL)-13 as well as airway responsiveness did not differ in COX-2 (-/-) mice and COX-2 (+/+) mice. However, sensitized COX-2 (-/-) mice had higher LTC4/D4/E4 and lower PGE2 concentrations compared with non-sensitized COX-2 (-/-) mice. The number of PAS/alcian blue-positive airway epithelial cells and serum IgE were elevated in COX-2 (-/-) mice. Nimesulide-treated mice showed augmented eosinophilic inflammation, LTC4/D4/E4 concentrations and mucous cell metaplasia. CONCLUSION: These data indicate that COX-2 deficiency augments allergic inflammation and mucous cell metaplasia.
